Diabetes Management Program

The Diabetes Management program is an application-based program for those participants who have been diagnoses with diabetes. This Program provides coverage of a glucose monitor with real-time feedback and emergency outreach services; testing strips and related supplies; and live coaching with a Certified Diabetes Educator within DSGI’s participating health plans to monitor eligible diabetic enrollees’ HbA1c and hypoglycemic events.
